100% WFH / Remote
Pediatric Practice
FULL-TIME | Approximately 40 Hours/Week | Monday–Friday
Bilingual English & Spanish Required
We’re looking for an experienced Pediatric Care Coordinator to manage well-visit recalls, care gaps, and insurance quality programs. This is a proactive coordination role focused on getting patients scheduled, closing care gaps, and ensuring complete documentation for reimbursement.
MUST HAVE
Previous experience in a pediatric practice or pediatric care coordination
Fluent in English and Spanish — written and spoken
HIPAA-trained
Experience with EHR/EMR task queues, patient scheduling, and clinical reports
Familiarity with pediatric well-visit schedules and common pediatric diagnoses
Excellent attention to detail and data-entry accuracy
Professional, compassionate communication with parents and caregivers
Able to work independently and learn new systems
KEY RESPONSIBILITIES
Manage well-visit recalls and schedule overdue patients
Coordinate Centene Continuity of Care Plus (CoC+) for MHS and Ambetter patients
Prepare, upload, and submit patient-specific agendas
Verify documentation and required ICD-10, CPT, CPT II, and HCPCS codes
Track CoC+ completion and payment reconciliation
Review care-gap reports across Medicaid managed care plans and other payers
Contact parents and caregivers in English and Spanish, including follow-ups for missed appointments
Maintain accurate trackers and report progress toward practice goals
STRONGLY PREFERRED
Pediatric nursing or comparable pediatric clinical experience
Knowledge of medical coding and Medicaid managed care
Experience with CoC+, HEDIS, or care-gap closure programs
Familiarity with Develo or payer provider portals
Previous remote healthcare support experience
TOOLS
Develo EMR, MHS Secure Provider Portal, other payer portals, Google Workspace, Slack, and HelpSquad’s secure virtual desktop.
Rate/offer determined based on experience and interview performance
