About the role
The Patient Care Coordinator will help manage the patient journey from initial scheduling through follow-up, while supporting case managers, patients, caregivers, and providers.
Key Responsibilities:
Coordinate and manage initial and follow-up patient appointments. Communicate with patients and caregivers regarding appointments and care coordination needs. Follow up on referrals, appointments, orders, and other patient care needs. Assist with obtaining and organizing medical records, reports, and referrals.
Ideal Candidate: Healthcare experience strongly preferred. Registered Nurses (RNs) are highly preferred.
Compensation & Benefits: Performance-based salary increase opportunity. 3-month probationary period. 14 days PTO after probation. Schedule: 8:30 AM - 5:30 PM Hawaii Time.
